These are the team types that have been sanctioned with VYSA for eligibility
to be able to participate. Failure to completely fill out the application will prohibit acceptance of your team into the tournament. All information must be completed.
NO TRAVEL TEAMS or TRAVEL PLAYERS are allowed to participate in the Mega Blast OR teams/players that have gone thru a tryout/invitation process of any type.
Recreational Team with Guest Players – A team that participates in a
recreational, house or intramural program for a club, league or association
plus. . . .no more than 5 guest players can be added to your regular season
roster for U12-U19 age groups and no more than 5 guest players can be added
to your regular season roster for U8–U10 age groups.
Recreational Team – A team that participates in a recreational, house,
or intramural program for a club, league or association with no guest
players added.
No All Star Teams are allowed in the tournament.
(All Stars Teams are identified as those teams that take players from more
than 1 team AND would involve more than 5 players included on a roster as
guest players.)

Red is the highest level of competition, white is the middle level of competition and blue would be the lowest level of competition for the Mega Blast.
Please note that the division that you request is not necessarily the division that you will end up in. After all applications are received for a specific age group, the tournament committee will look at the group as a whole and then determine by a number of factors where each team should be placed. |

PLEASE READ THE ENTIRE STATEMENT BELOW AND THEN CHECK THIS BOX. CHECKING THE BOX INDICATES THAT YOU ARE AWARE OF ALL SITUATIONS WITH THE MEGA BLAST THAT ARE IMPORTANT TO BE AWARE OF. (It is suggested that you print off this information so that you will have it for reference prior to the tournament weekend.)
I understand that I can add no more than 5 guest players to my "recreational" roster and they must be noted as Guests on my final roster that I submit at registration the night before the tournament and I understand that I cannot add any additional players to my roster after checkin Friday evening prior to the tournament without the permission and necessary tournament paperwork from my league administrator AND approval of the tournament committee. If I knowingly add players that have not been thru the above approved process, my team will have to forfeit any games played and then be disqualified from the tournament.
I also understand that I must submit recreational player cards that have been signed by both my league administrator and the player and an original and 2 copies of my tournament roster that have also been approved by the league administrator and that all guest players are properly noted and designated on such roster and also a current medical release form is provided at checkin for each player on the tournament roster. Please get in touch with your league administrator immediately to notify them that you are participating in this tournament and will need Recreational Player Pass Cards along with a Tournament Roster. All player cards and the tournament roster, MUST be signed off on by the league administrator from your club.
I understand that I will keep with me at all times during the tournament weekend the approved tournament roster, player cards and medical release forms in the event that they need to be verified at any point during the tournament.
I also understand that if requested, I will provide picture ID's of any player in question on my tournament roster. (All age groups U16 and older should have their driver's license with them during the tournament weekend in order to provide identification and proof of date of birth if needed.)
I also understand that if I am the Head Coach of more than one team that I will need to insure that I have an Assistant Coach in place for each team because conflicts of the different teams cannot be accommodated. Because of the volume of teams that we service, it is not possible to work out coaching conflicts with multiple teams and fields.
I understand that once I submit this application that I must make contact and make a payment either by check or credit card in order to secure my team's spot in the 2010 Mega Blast. I understand that until payment is received, my team stands the possibility of NOT being scheduled in the tournament and stands to go onto a wait list until a spot opens up in my age group.
Note: Just because you register as a Rec team with no guests does not mean that you will be placed in a lower bracket competition.

All teams must submit their Fall Record. If you are not playing as a fall league, you must give an explanation why below and detail events that this group participates in. Even if you were a new team for the Fall 2010, you must note your record or you stand to be placed in the highest division.

Application AND payment must be RECEIVED
by October 15, 2010 in order for application to be considered
complete. Entry fees cannot be refunded after final acceptance.
Once this application is submitted, in order to be complete, you will need to contact the office at (804) 744-8505 in order to make payment by credit card. The application will not be accepted without payment. Payment is expected at the time application is made in order to confirm your commitment to participate in the 2010 Mega Blast. (If payment is not received by the time the application is processed, we will be contacting you by phone for a credit card so that we can process your payment. Please be prepared to have that information ready in order to secure your spot in the Mega Blast.)
Please understand that by submitting an application states your team's intent to participate and teams are then immediately scheduled into the tournament system. If you register online, it is expected that your team will be participating. By making payment this confirms your commitment to participate in the event. If you should pull out at the last minute because of lack of commitment by your team, please understand that it will impact other teams. Please be sure that you are ABLE to field a team before you register. Other teams are counting on you to fulfill your commitment by fielding a team. If you should pull out at the last minute, it impacts more than just your team.
In the event that we are unable to accept your team, the payment amount will be refunded. By calling the league office, we will be able to run a credit card payment for you and thus secure your spot for the 2010 Mega Blast. Please call between 9am and 1pm with the information. Please note that if you register on October 15 and then try to call, the volume will be high. Please register in advance and avoid waiting and a possible late fee.
Either ONE check for your team or ONE credit card payment will finalize your application. Please DO NOT send separate checks by each player. We realize that you are dealing with parents that may be making decisions whether to participate or not and payments may be forthcoming from your team to you. However, in order to properly and professionally schedule this tournament, it is imperative that payment accompany your registration application. Payment received will indicate a complete registration and your commitment for the tournament.
Applications and payment received after October 15 deadline will be subject to a $25 late fee.
Every attempt will be made to complete the tournament in its entirety. In the event of circumstances that we cannot control and the tournament has to be altered in any way, there will be NO refunds.

for $350 (U8, U10) or $400 (U12 and older age groups) by October 15
- Legible copy of preliminary roster WITH dates of birth to determine correct age group competition. (Preliminary roster must contain names of players and dates of birth of ALL players to determine correct age bracket.
